Youth Participants
We only require the number of youth you are sending to Spirit Day; there is no need to type in each participant’s individual information. You will enter in chaperone information further down in this form.

1 chaperone is required for every 8 youth.
Where applicable, "Youth Leaders" who are under the age of 19 years old should be included in the above number and in the chaperone to youth ratio.

This is the person who will pick up the Registration Package at Chandos Pattison Auditorium and is responsible for the group on the day of the event. (ie. having all important information regarding the group).
