This application is intended to help parishes who would otherwise be unable to continue their FORMED subscription to receive the necessary funding to do so.
If you receive the subsidy, you will still need to go through the normal automatic renewal with FORMED at the full price (in US funds). We will then issue a cheque back to your parish with the subsidy amount (in Canadian funds). Please allow a few weeks for the cheque to be received back at the parish.
Subsidy amounts:
Parishes with greater than 400 households are eligible for a $625 Canadian subsidy
Parishes with less than or equal to 400 households are eligible for a $325 Canadian subsidy
